How to Create Charts and Consultation Notes for Patients

How To Create A Chart / Consultation Note For A Patient
This guide will walk you through the process of creating a chart or consultation note for a patient in the EMRG system.
Note: Clinics can customize their charts under "Clinic Management" > "Chart Management". This guide focuses on how to fill out charts once they have been created and configured.
Step-by-Step Process
1. Navigate to Patients
Click "Patients" in the side navigation bar to access the patient management interface.

2. Search for Patient
Click the "Search by patient name or ID" field to find the specific patient you want to create a chart for.

3. Access Appointments
Click "Appointments" to view the patient's appointment history.

4. Select Appointment
Select the specific appointment of interest from the appointments list.

5. Create New Chart
Scroll down and click "New Chart" to begin creating a consultation note.

6. Select Chart Type
Click "Consultation and Treatment Note" or the name of your clinic's custom chart template.

7. Fill Out Treatment Note
Complete all required fields in the treatment note form according to your clinic's documentation standards.

8. Save as Draft (For Non-Physicians)
IMPORTANT: If you are NOT the overseeing physician in charge of signing charts, then you should click "Save as Draft".

9. Mark for Review
Then you can open that same chart and click "Mark for Review" which will then indicate to your overseeing physician that the chart is ready for their review and signature.

10. Confirm Mark for Review
Click "Mark for Review" to change the status of the chart to "Needs Review" which will then indicate to your overseeing physician that the chart is ready for their review and signature.

11. Sign & Submit (For Physicians)
If you are the overseeing physician, then you can click "Sign & Submit".
Note: You can send your overseeing physician this article: How to Review and Sign Charts to give them simple instructions on how to review and sign the charts that are marked as "Needs Review."

12. Review All Charts
All charts and their statuses can be reviewed if you click "Charts" on your side navigation bar.

What Happens Next
The completed chart/consultation note will be saved to the patient's record and associated with the specific appointment. The note will be available for future reference and can be accessed through the patient's chart history.
For non-physician Providers
- Charts saved as "Draft" can be edited and completed later
- Use "Mark for Review" to notify physicians when charts are ready for signature
- Charts marked for review will appear in the physician's "Needs Review" filter
For Physicians
- Charts marked as "Needs Review" will appear in your charts list
- Use the "Sign & Submit" button to finalize charts
- All signed charts become part of the permanent medical record
