This guide will walk you through the process of creating custom forms in the EMRG system, including all available form components and their uses.
đź“‹ Form Component Behavior and Common Uses
Subtitle - Break up the form into sections with clear headings (e.g., "Personal Information" or "Payment Details").
Text - Collect short responses like names, email addresses, job titles, or comments.
Date - Capture specific dates such as birthdates, appointment times, or event deadlines.
Checkbox - Confirm user consent or agreement (e.g., "I agree to the terms and conditions") or allow optional selections like subscribing to a newsletter.
Select (Dropdown) - Let users choose a single option from a list, such as selecting a country, role, or payment method.
Multi-select - Allow users to choose multiple applicable options, like skills, interests, or preferred contact methods.
Radio (Single Choice) - Present mutually exclusive choices, such as "Yes" or "No," or choosing one preferred plan or service.
Text Block - Display instructions, disclaimers, or context using formatted text—ideal for section introductions or legal language.
Step-by-Step Process
1. Access Form Templates
Click "Form Templates" in the navigation to access the form management interface.

2. Create New Form
Click "Create New Form" to begin building a new form template.

3. Enter Form Name
Click the "Form Name" field to enter a name for your form.

4. Type Form Name
Type the name of the form that will be displayed to users.

5. Create Form
Click "Create Form" to initialize your new form template.

6. Edit Form
Click "Edit" to begin adding components to your form.

Click "Add Component" to add a new field to your form.

8. Select Field Type
Click which field type you would like to add to your form.

9. Configure Field
Type or enter data for the chosen field type (label, options, etc.).

10. Set Required Status
Click this button to toggle whether the field is required or not for the patient to fill out to complete the form.

11. Save Component
Click this button to save the component configuration.

Click "Save" to save your form with the new component.

Click "Create Component" to continue building your form with additional fields.

Your form list should look like this with all your configured components.
